What should you do?
Please fill out the form and return it to us.
If you didn’t receive a letter/form and wish to participate, please get a form from the office, fill it out; and return it.
If you did receive a letter and do not wish to participate, please return the form in the envelope, and we will not contact you again.
CHY4 – Annual Declaration is only for one year and is available in the parish office.
We look forward to hearing from you as soon as possible. Many thanks.
Many parishioners have filled out the CHY3 form, so they won’t be hearing from us this year.
